As a Wingate patient, you may benefit from additional care with transition services that provide the critical support needed to keep you safe and healthy after you return home. The transition services offered by Wingate at Home are designed to address the primary reasons for readmission, such as confusion with medications and overall health management. Wingate at Home offers peace of mind by providing the support that patients and caregivers need during periods of transition.
The Transition Service provides:
- Medication Management Services: In depth review of your medications from before admission and after discharge, and coaching on how to simplify and safely manage your medications
- Health Management Support and Coaching: Coaching on how to safely manage your health
- Care Coordination: Recommendation and guidance for resources in and around your home such as transportation, home services and community wellness programs
